All Medicare Beneficiaries are eligible for Annual Wellness Visit, at no out of pocket cost to patient, no deductibles, co-pays or coinsurance apply.
The Goal of Annual Wellness Visits is to detect risk factors and illnesses. resulting in prevention, early intervention and treatment.
Medicare does provide for an Initial Patient Physical Exam (IPPE). Code GO402
Initial Patient Physical Exam is a patient's Welcome to Medicare Exam, which is required to occur within the initial 12 months of the beneficiary's enrollment in Medicare. No out of pocket cost to the patient.
Initial Annual Wellness Visits: Code GO438
Initial Annual Wellness Visit is allowed once in a patient's lifetime and occur when the patient is outside of the initial 12 month of Medicare enrollment and has not received an Annual Wellness Visit.
Subsequent Annual Wellness Visit: Code GO439
Subsequent Annual Wellness Visit occur every 12 months. The Personalized Prevention Plan Services promotes prevention via patient education, screenings and monitoring.
Advanced Care Planning: Code 99497
Advanced Care Planning is a separate 30 mins minimum add on component of the Annual Wellness Visit, which includes explanation and discussion of the Advanced Directives regarding the patient's wishes surrounding medical treatment if the patient is unable to speak or make decisions regarding their medical care.

Geriatric Annual Wellness Visit
Geriatric Annual Wellness Visit is a comprehensive face to face assessment, for patients 65 years and older. The GAWV is a face to face assessment with a member of your healthcare team. The face to face interaction allows the patient to build trust with the healthcare team member while affording the patient more time to express his/her health concerns. Patients will be encouraged to become proactive in identifying health goals and directing their personal journey of wellness.
